Village Public Health Health Coaching Terms and Conditions

Thank you for choosing Village Public Health for your health coaching needs. Please carefully review the following terms and conditions that govern our group and one-on-one coaching services.

​

1. Agreement to Terms
Participating in Village Public Health coaching services acknowledges that you have read, understood, and agree to be bound by these terms and conditions.

​

2. Coaching Services
Village Public Health offers health coaching services, including but not limited to:

  • One-on-One Health Coaching

  • Group Health Coaching

  • Mental, Physical, and Emotional Wellness Coaching

  • Goal Setting and Accountability Sessions​
     

Our services are designed to support and guide individuals toward their health and wellness goals; they are not a substitute for medical advice, diagnosis, or treatment from a licensed healthcare provider.

3. Confidentiality
All information shared during coaching sessions is kept confidential and will not be disclosed to third parties without written consent, except as required by law.

​

4. Participation and Expectations
To ensure the best outcomes, participants are expected to:

  • Attend scheduled coaching sessions on time.

  • Actively engage in the coaching process.

  • Complete assigned tasks or action steps.

  • Communicate any concerns or questions to their coach in a timely manner.
    ​

5. Scheduling and Cancellations

  • All coaching sessions must be scheduled in advance.

  • Cancellations or rescheduling require at least 24 hours' notice. Failure to provide timely notice may result in forfeiture of the session without a refund.

  • Village Public Health reserves the right to reschedule sessions due to unforeseen circumstances.
     

6. Fees and Payment

  • Coaching fees must be paid in full prior to the start of services.

  • Payment plans may be available upon request.

  • Refunds are not available for completed sessions. Requests for refunds on unused sessions will be considered on a case-by-case basis.

7. Code of Conduct
Participants are expected to maintain respectful and professional behavior during sessions. Village Public Health reserves the right to terminate services without refund if a participant engages in inappropriate or disruptive behavior.

8. Health Disclaimer
Our coaches do not provide medical advice or treatment. Always seek the guidance of a qualified healthcare professional before making any changes to your health regimen.

9. Liability Waiver
Participants understand and agree that Village Public Health, its coaches, and affiliates are not liable for any personal injury, emotional distress, or other damages resulting from participation in the coaching program.

10. Termination of Services
Village Public Health reserves the right to terminate coaching services at any time due to non-compliance with these terms, failure to make payment or other reasonable causes.

 

11. Changes to Terms and Conditions
Village Public Health may update these terms and conditions from time to time. Participants will be notified of any significant changes.
